When you get the hookshot from the graveyard either play the Minuet of the Forest (warp song, if you have it) or go through the Sacred Forest Meadow (which has big enemies). Go where Saria used to sit. You'll find a broken staircase and a tall tree. Hookshot the tree. Now you can go inside the Forest Temple.
Go to the Kokiri Forest, and go through the Lost Woods until you reach the Sacred Forest Meadow. There, go through the maze, killing all the Moblins with your Hookshot, until you reach the area where you met Saria as a child, where Sheik will teach you the Minuet of Forest. After the cutscene, use the Hookshot on the tree above you to go onto the steps which lead into the Forest Temple.
