Night Elf druids can summon Lunaclaw at the Moonkin Stone in the Moonkin Caves, directly east of Auberdine in Darkshore.
Tauren druids can summon Lunaclaw at the Moonkin Stone west of Camp Taurajo in The Barrens.
When Lunaclaw is defeated, the druid can learn bear form from the Lunaclaw Spirit.
However the quest was no longer available after a patch BEFORE the cataclysm patch, and is now a trainable spell at level 15.

NOTE: In the future expansion Cataclysm (currently in Beta), they are switching the levels at which you get cat and bear form. Cat form will be at level 10 and Bear form will be a little higher than that. Also they will be removing the Tree of Life form as a passive form making it a cooldown based ability.
