Tier 6 is gear from the Burning Crusade expansion. You get tokens from Black Temple etc. and you cash the tokens in at Shattrath City. Depending on whether your aldor or scyer, you have to go to each of those rises I believe it's called, in the city. Look around up there and you'll find the vendor!

Right now in the newest patch the highest is 7.5. Plus tier can be bought not in just one set. For example warriors can get dps or tanking gear
