There's no cheat, but with enough Lifetime Achievement Points you can buy "Fertility Treatment' which will heighten your chances of having either twins or triplets.
Note: It works better if both parents have this.

To have three kids, get a male sim and a female sims to relax in a double bed and click try for baby. Your sim's pregnancy will last three sims days. Go to the cheat box: Ctrl + Shift + C and type in forcetwins. Unless you install a specific cheat you can't have triplets. You can try find that hack/mod on modthesims2.com. Happy simming!

"forcetriplets" - without the ""for quadruplets.. "quadforce" - without the ""if they dont work try it in capital letters :)
