Since the introduction of the JAG (Jagex Account Guardian), it is no longer possible to setup recovery questions as it worked previously. We are now required to setup JAG for any sort of "recovery" security to be enabled on our accounts, so you'll have to setup JAG.

Recovery questions are highly recommended as they add an extra layer of security to your account. If your account is compromised you will have a better chance of getting your account back as fast as possible. Recovery questions should be questions that you will remember, if you think you might forget the answers right them down on a piece of paper and put it in a safe place.
Why can recovery questions and answers be changed in runescape without using existing recovery questions and answers?
Recovery Questions are made to allow getting your account back more easy. This happens a lot, but all you can do is keep trying and making sure you can provide as much detail and accurate information as you can, to prove that you are the owner of the account.
How do you fixed your bugged RuneScape account?
To recover your password on Runescape, on the homepage click on 'Account' on the navigation bar at the top of the page, you then click on 'account management' when you will then be linked to another page.
