Yes. People on The Sims 1,2, and 3 do get sick. There are many ways for them to get sick too; you can get a girl or guy pregnant where they'll just throw up once or twice, if you have a vegetarian Sim and have them eat meat they will get sick, they can eat old rotten food, or you can force sickness upon them by opening the cheat console (Ctrl+Shift+C) and type 'testingcheatsenabled on' and hold shift and click on your Sim and click spawn I do believe and then you'll find much more stuff and you'll find something saying sickness tester or something like that, or just don't take care of them and they could even die. I'm sure there are many other ways to get your Sims sick but here are just a few.

If you are talking about installing a Sims 2 expansion to the Sims 1, it is impossible. You can only install the Sims 2 expansion packs into the Sims 2, not The Sims 1 or 3.
