I do not know what "action" you're talking about, but I do know that people are NOT asleep during anesthesia. They are in a specific form of unconsciousness that resembles coma but which is reversible.

Pretty much the same way it's done now, except with rusty saws, a lot of whiskey, and a great deal of blood. Pre-anaesthesia medical practices were not fun. Usually, the treatment was worse than the condition. Amputations were done routinely. You would not have wanted a broken leg back then.
