To correct the above post, 4 bronze oak leaf clusters indicates 5 total awards of a single medal (the initial award +1 OLC for each additional award). Starting with the 6th award, a silver oak leaf cluster is worn instead (silver OLC indicating 5 previous awards). The 7th will add another bronze OLC, and so forth. Caveat, this is for the Army. The Naval services use stars instead of OLCs to indicate previous awards.
As with all military awards, you have the bar, and an additional cluster for each medal awarded after the first. So, you have the ribbon and four clusters.

If you happen to be awarded one more Army Achievement Medal you would replace the Bronze Oak Leaf Clusters with a single Silver Oak Leaf Cluster, signifying 5 subsequent Medals awarded.
Your fifth award would include the ribbon plus four bronze oak leaf clusters. Upon the sixth award you would remove the four bronze oak leaf clusters and replace with one silver oak leaf cluster. In reality, you would get a new ribbon and put one silver oak leaf cluster on it, so the holes from the removed clusters do not show.
An award of 9 Army Achievement Medals would be shown by the original medal pinned with a silver oak leaf cluster (for 5 awards) and 4 bronze oak leaf clusters.
