From everything I have been told and read, the three things you should be sure to have to go to Cuba as a Canadian are: your passport, a tourist visa (some agencies provide this in your package), and 25 Cuban pesos for the departure fee at the airport when you leave Cuba. The tourist visa can also be obtained from a Cuban consulate but can take a week to receive depending on the method you choose for applying (postal applications will obviously take more time).

Passports from US citizens crossing Canadian and Mexican borders by land or sea in have been required since June 2009. Yes, you need a passport to go to any other country that's not your own
Ideally Yes. Since visiting Canada if you are not Canadian is an international Travel. This means the Immigration will check you if you are eligible to Enter. This calls for a proof of your citizenship and identity both of which a passport fulfills. It is safer if not preferred to travel with you passport with you.

Yes.

A valid Canadian passport should be carried for all visits outside Canada. However, Canadians are only required to be in possession of valid official photo identification (such as a driver's licence) and proof of citizenship (such as a birth certificate) for travel to Jamaica. An onward or return ticket and proof of sufficient funds is also required.

Cuba requires a passport. The passport should be valid for at least 6 months from the date of entry into Cuba.
