The first Geneva Convention was held on August 22, 1864 in Geneva, Switzerland. The 12 countries involved were:
Baden, Belgium, Denmark, France, Hesse, Italy, Netherlands, Portugal, Prussia, Spain, Switzerland and Wuttemberg
In total, there have been four Geneva Convention treaties, and three other Protocols. The first Geneva Convention met in 1864 in Switzerland, with twelve nations participating: France, Belgium, Denmark, Italy, the Netherlands, Portugal, Spain, Prussia, Württemburg, Switzerland, the Grand Duchy of Hesse, and the Grand Duchy of Baden. Other nations soon signed the treaty. The Second Convention was held in 1906. The Third Convention was held in 1929. And the Fourth Convention was held in 1949. The Protocols were amendments to these Conventions in later years. The first and second Protocols were added in 1977, and the third added in 2005.

On December 1, 1959, representatives of twelve nations sat down together and signed the Antarctic Treaty in multiple languages. The languages are English, French, Russian and Spanish. There is no indication that any one of these countries was 'first'. All twelve nations are considered 'original signatories'.

Twelve representatives of each founding country signed the charter of CERN at the 29th of July 1953 in Paris. Beforehand CERN existed only as an idea, a provisional Organisation with a provisional Council, composed by eleven European governments. The new Council determined Geneva/Switzerland as the center of CERN. 1955 was the laying of the first stone.
12 states went to the Constitutional Convention (also known as the Federal Convention, the Philadelphia Convention, the First Constitutional Convention, and the Constitutional Convention of 1787). Rhode Island didn't send delegates because it opposed any revision of the Articles of Confederation and it did not believe the national government had the right to interfere in the affairs of a state.All the states expect Rhode Island were represented at the constitutional convention. Because it had been self governed for over 140 years. And the state was reluctant to give up power to a central government.
NATO was created by twelve countries in 1949 as part of the mutual defense against the Soviets. Fear of nuclear warfare by the Soviets led to this group decision.
