Well if it's East 28th St. you take the #6 train to 42nd St. uptown and change to the #4 train uptown to E.161st St. Yankee Stadium.
Take the Uptown E train (of the A-C-E, the blue line) from 34th Street-Penn Station to Lexington Avenue-53rd Street. The Lexington Avenue-53rd Street Station is connected to the 51st Street Station on the 6 train (of the 4-5-6, the dark green line).Transfer to the Uptown 6, and take the Uptown 6 to 68th Street-Hunter College (at Lexington Avenue). Then walk east to York Avenue.
Take the Uptown 6 local train (of the 4-5-6, the dark green line), 1 stop, from 77th Street (at Lexington Avenue) to 86th Street. Transfer at 86th Street to the Uptown 4 express train, and take the Uptown 4 to 161st Street-Yankee Stadium.Or, you can just walk up Lexington Avenue to 86th Street and get the Uptown 4.
It depends on whether you're going to West 66th Street or East 66th Street. To West 66th Street, you can take the Uptown 1 train (of the 1-2-3, the red line) from 34th Street-Penn Station to 66th Street-Lincoln Center (at Broadway). To East 66th Street, you can take the Uptown E train (of the A-C-E, the blue line) from 34th Street-Penn Station to 53rd Street-Lexington Avenue, and transfer at 53rd Street-Lexington Avenue to the 51st Street-Lexington Avenue on the 6 train (of the 4-5-6, the dark green line), and take the Uptown 6 to 68th Street-Hunter College (at Lexington Avenue).
Take the Uptown E train (of the A-C-E, the blue line) from 34th Street-Penn Station to Lexington Avenue-53rd Street. This station is connected to the 51st Street Station on the 6 train (of the 4-5-6, the dark green line), so you can transfer without exiting the subway system. Transfer to the 6, and take the Downtown 6 to 33rd Street, 28th Street, 23rd Street or 14th Street. All of these stops are on Park Avenue South.
You have several options. Which one to choose depends upon which part of the Bronx you are going to. See the Related Link below for a complete New York City subway map.Option 1: The 2 trainYou can take the Uptown 2 train (of the 1-2-3, the red line) straight from 34th Street-Penn Station to the Bronx.Option 2: The 5 trainTake the Uptown 2 train from 34th Street-Penn Station to 3rd Avenue-149th Street, and transfer there to the Uptown 5train.Option 3: The 4 or 5 trainsTake the Uptown 2 train from 34th Street-Penn Station to 149th Street-Grand Concourse, and transfer there to the Uptown 4 or 5.Option 4: The 6 trainTransfer is not available from the 2 train to the 6 train, so if the 6 train is the closest train to your destination, then you will have to take the Uptown 2train from 34th Street-Penn Station to 149th Street-Grand Concourse, and transfer there to the Downtown 4 or 5. Take the Downtown 4 or 5 to 125th Street in Manhattan, and transfer at 125th Street to the Uptown 6train.You could also take the Uptown 1-2-3, one stop, from 34th Street-Penn Station to 42nd Street-Times Square, and transfer there to the S shuttle train (color grey) east across 42nd Street, from Times Square (West 42nd) to Grand Central Station (East 42nd). Then, either transfer at Grand Central to the Uptown 6(which is a local train) or transfer to the Uptown 4 or 5 express trains to 125th Street, and then transfer at 125th Street to the Uptown 6.Although the first option requires you to go Uptown, then Downtown, then back Uptown again, it will still probably take less time than the second option. The 6 is a local train, so there are many, many stops from Grand Central Station to the Bronx. As mentioned above, you can transfer at Grand Central to the Uptown 4 and 5 express trains to 125th, and then transfer at 125th to the Uptown 6. But then you'll have to make three transfers. The first option requires you to make only two transfers.Option 5: The B or D trainsTake the Uptown A or C trains (of the A-C-E, the blue line) from 34th Street-Penn Station to 59th Street-Columbus Circle, and transfer there to the Uptown Bor D trains (of the B-D-F-M, the orange line).

The Downtown (Manhattan-bound) Dskips the 161st Street-Yankee Stadium stop from 6:15 to 9 AM on weekdays, and the Uptown (Bronx-bound) D skips the Yankee Stadium stop from 4 to 6:45 PM on weekdays.

I am confused by your question, because the D train (of the B-D-F-M, the orange line) does not stop at 79th Street. It's an express train, and it runs straight from 59th Street-Columbus Circle to 125th Street. The other thing I'm confused by is what you're actually asking: "where do I transfer?" You transfer from one train to another inside the station. I would think you would understand that already. The only train that stops at 79th Street is the 1 train (of the 1-2-3, the red line) at West 79th Street and Broadway. And it is the only train that stops at that station. There is no other train to transfer to.If you are sure that you're supposed to be taking the Dtrain, then: take the D train to 7th Avenue-53rd Street. Transfer there to the Uptown (Queens-bound) E train (of the A-C-E, the blue line). Take the E train 2 stops to 53rd Street-Lexington Avenue. This station is connected to the 51st Street Station on the 6 train (of the 4-5-6, the dark green line). Take the Downtown 6 to 14th Street-Union Square.
The only Number 6 train you may be referring to is the MTA Subway number 6. This train line does not go to NYC Penn Station, but does go to Grand Central Station. The best suggestion is to take the MTA "S" train (for Shuttle) that gives you a very fast trip from Penn Station to Grand Central. There you will find the number 6 train.
