Some people use a "punch" cutter that gives you a round hole in the end of the cigar cap. Waste of time. Cut the whole end. Much better draw.
The "head" is the closed end of the cigar. You light the "foot". You will have better luck using a cigar cutter. But, to each his own. I saw a friend use a tooth pick to punch a hole in the end of a fine cigar the other night instead of using my $50 Xikar cutter???
By clipping a cigar, you are simply removing the covered end in order to allow for air and smoke to be drawn through the cigar. You don't need to take very much off. A Cuban cigar roller once taught me to simply remove the cap of a cigar with my finger nails. This can normally be accomplished successfully, but you've got to be careful not to tear the wrapper.

If you have a tear in the "head" end, the end you cut, you can lick it to attempt to hold it together and to keep it from sucking air. A tear in the "foot" end... if it is not affecting your draw, just smoke through it. You can cut it, but more than likely when you have a problem it is an indication of storage problems. A split that starts near the foot and continues just ahead of the burn can be a sign that your humidity is too low. It is also true that you can get a bad one every once in a while.
