A PUK code needs to be entered when you have had three tries and could not enter the correct sim pin. The PUK code will be written on your sim card
Call your T-mobile network
You will have to contact AT&T and explain to the operator that you need the PUK code for your phone. S/he will ask you what type of phone you own and the IMEI number for your phone. (The IMEI number can be found under the battery in your phone.) The operator will then give you the PUK code. Make sure you record the PUK code somewhere because you will have to enter it into your phone any time your phone is powered off from now on.
Stop messing with it immediately. Only your service provider can provide you with your PUK. It is not on your box and it is not on the card that the SIM card was punched out of. The PUK is unique to each SIM card so nobody on here can give you one (well, I guess they can but it will not work) and if you put in an incorrect code 10 times you will "fry" the SIM card and will have to purchase a new one for around $25.00-$30.00. Call your service provider and explain that your phone is asking for the PUK and they will give it to you and will walk you through putting it in and getting your phone working again. I do know that AT&T also allows you to get your PUK on their website but I do not know if others do that or not.
