New York City has the largest population of Italian/Sicilian Americans and always has, followed by Philadelphia, Chicago, Boston and Pittsburgh.
With exceptions for the largest cities in western U.S. states, most Italians originally settled on or near the east coast. This still holds true today.
It's different according to the time when they arrived and where they were from. For instance, immigrants in the 18th century would have had few places to go beyond the 13 colonies (the natives were still, well, ALIVE at this point). European immigrants largely remained in the urban population centers of the east. Later on, however, some groups established communities in the west, such as the Germans in Texas and Scandinavians in the Midwest. Asian immigrants tended to come by way of the pacific, hence the reason that so much legislation was enacted on the 19th century against Chinese and, to a lesson degree, Japanese in California. That's all I know, as someone who is by no means an expert or even that well educated on the subject: hopefully someone who has read up on the subject can improve this answer.
