For individuals on the path to transition (a person with TSism rather than TGism), a therapist can supply a letter that will protect an individual from legal repercussions, lest their appearance cause temporary problems with other restroom users, local security, etc.
Sadly, such a letter does not protect them from the often vicious comments from other restroom denizens, and will lead to shying away from restrooms, or using the restroom of the wrong sex, essentially misgendering themselves in the process, also a traumatic experience.
Family and unisex restrooms are a boon, but the most common practice is to go before one leaves the house, whenever a restroom presents itself, and in known friendly areas, and holding it in when an appropriate restroom does not present itself. Some even use the restroom outside, but you must be careful if you do that since you could be arrested and perhaps even branded a sex offender.
It should also be noted that in many states, using the "wrong" restroom without some form of proof of transition is illegal, even if the individual is presenting as the appropriate gender at the time. Restrooms are one of the most uncomfortable parts of a transgendered or transsexual person's life.
