Definition

Ehlers-Danlos syndrome is a group of inherited disorders marked by extremely loose joints, hyperelastic skin that bruises easily, and easily damaged blood vessels.

Causes, incidence, and risk factors

There are six major types and at least five minor types of Ehlers-Danlos syndrome (EDS).

A variety of gene mutations (changes) cause problems with collagen, the material that provides strength and structure to skin, bone, blood vessels, and internal organs.

The abnormal collagen leads to the symptoms associated with EDS. In some forms of the condition this can include rupture of internal organs or abnormal heart valves.

Family history is a risk factor in some cases.

Symptoms

Symptoms of EDS include:

  • Double-jointedness
  • Easily damaged, bruised, and stretchy skin
  • Easy scarring and poor wound healing
  • Flat feet
  • Increased joint mobility, joints popping, early arthritis
  • Joint dislocation
  • Joint pain
  • Premature rupture of membranes during pregnancy
  • Very soft and velvety skin
  • Vision problems
Signs and tests

Examination by the health care provider may show:

  • Deformed surface of the eye (cornea)
  • Excess joint laxity and joint hypermobility
  • Mitral valve prolapse
  • Periodontitis
  • Rupture of intestines, uterus, or eyeball (seen only in vascular EDS, which is rare)
  • Signs of platelet aggregation failure (platelets do not clump together properly)
  • Soft, thin, or very stretchy (hyperextensible) skin

Tests performed to diagnose EDS include:

  • Collagen typing (performed on a skin biopsy sample)
  • Collagen gene mutation testing
  • Echocardiogram(heart ultrasound)
  • Lysyl hydroxylase or oxidase activity
Treatment

There is no specific cure for Ehlers-Danlos syndrome. Individual problems and symptoms are evaluated and cared for appropriately. Frequently, physical therapy or evaluation by a doctor specializing in rehabilitation medicine is needed.

People with EDS generally have a normal life span. Intelligence is normal.

Those with the rare vascular type of EDS are at significantly increased risk for rupture of a major organ or blood vessel. These individuals, therefore, have a high risk of sudden death.

Complications

Possible complications of Ehlers-Danlos syndrome include:

  • Chronic joint pain
  • Early-onset arthritis
  • Failure of surgical wounds to close (or stitches tear out)
  • Premature rupture of membranes during pregnancy
  • Rupture of major vessels, including a ruptured aortic aneurysm(only in vascular EDS)
  • Rupture of a hollow organ such as the uterus or bowel (only in vascular EDS)
  • Rupture of the eyeball

The lab can tell the difference between oxy and Vicodin, even when people metabolize differently. If the metabolizing is the case, the oxy will only show up as a trace. You can ask the doctor to try you on something else to see if that is the case.

Vicodin is not generally given for pain management for any of the conditions listed, as it would be the wrong type of pain medication. RA and Lupus are both auto-immune diseases, which cause large amounts of inflammation. NSAIDs or immuno-suppressants are usually used instead.

Double jointed is a condition where one's joints are capable of moving in a wider range of motion than people who are not double jointed. The medical term is hypermobility. The adjective is hypermobile. How much one is hypermobile is determined by measurements of the Beignton Scale. Named for a physician who took the time measure such things. Two examples are how well one can touch their thumb to their arm just below their wrist. Second one is from a standing position placing one's hands palms flat on the floor, without bending knees. The cause depends on what is causing the condition. Some genetic causes include Marfan syndrome, Morquio syndrome ( a type of dwarfism) and Ehler Danlos hypermobility type.

There is currently no cure for it, although many doctors do think that a lot more people have it than there are currently known to be, as Ehlers-Danlos can sometimes be unnoticed for years, or even someone's whole life, even today. It can be managed- for example, I am a person with type II (also known of as the Classical type- I and II are NOT the same, and they shouldn't be treated as such), and so instead of wearing skirts and dresses with thin tights (or, shudder, nothing at all) underneath, I wear thicker leggings and don't climb trees. One can't do anything for the dislocations, and the internal types of EDS can't be helped. One can do family planning with a 50% chance (I think...) of it being correct, but if you are against it (can't blame you, I kinda am too), you can't prevent your kids from getting it and having them be yours genetically. People are currently working on finding a cure, although it is almost as close as some of the cures for cancer.
