One of the BEST ways to keep a lizards nails in check - is to provide a small area of rough slate on the floor of the vivarium - that way, when the lizards walks on it, the nails will naturally be filed down.
It can frequently occur if they haven't been able to entirely shed skin from their toes.
The skin constricts the toe as the toe continues to grow, cutting off circulation and causing the toe fall off. It does heal quickly, although it doesn't regrow.
They need an environment with sufficient humidity to help them shed their skin properly.
