The words of baptism, which are said as water is poured onto the person being baptised, are "I baptise you in the Name of the Father, and of the Son, and of the Holy Spirit."
This is in keeping with 'The Great Commission' of Jesus as recorded in Matthew 28:19:
Go therefore and make disciples of all the nations, baptizing them in the name of the Father and the Son and the Holy Spirit.
Anyone baptised with this formula is considered baptised by all Christian denominations. If they join a different church, they do not need to be baptised again. Anyone can baptise, such as a nurse or the mother or father in the case of a newborn baby near death or deceased (this would be referred to as conditional baptism, whose effectiveness would be known only to God).
