In today's world, the vast majority of national governments are called democratic ones. Other types are called monarchies and some still are classified, or indeed call themselves communist governments. Another type, as the one in Iran is, are a religious government.
Generally speaking, a country that is ruled by persons who have been elected by a free type of election process where more than one party places candidates for elected office is called a "democratic government". In some cause, and I emphasize "some" they are called "democratic republics".

There were probably no governments in the Middle Ages we would call true democracies, but there were a number of towns and cities, called communes and free cities, that had republican governments, some of which approached true democracy. Some of these were within monarchies, and others were independent.
