Cabinet Ministers are primarily elected as Members of Parliament, and subsequently appointed by the Prime Minister as members of the cabinet.
The Council of Ministers of Andhra Pradesh are a part of the executive branch in India. They have 25 appointed ministers.
Ministers of the Crown are not elected. They are appointed by the Governor General on the advice of the prime minister. As ministers are responsible to members of the elected House of Commons, ministers are generally appointed from among members of the Government caucus in the Lower House. It is not unusual, however, for one or more ministers to be appointed from the Senate.
This depends on the system and the country. Generally, Prime Ministers are chosen by the elected representatives of an assembly, or appointed by the President and possibly confirmed. New Prime Ministers can occur every year or every 15 years.
Members of the Senate, and ministers in Cabinet, are similar in that both are appointed by the Governor General on the advice of the prime minister.

In terms of religion, ministers are appointed by a community of believers. "Technically speaking, only God can ordain ministers. It is God who calls and ordains people into ministry. The irony is that man still has to acknowledge this calling to make it "legal". Therefore, from a legal standpoint, religious organizations can ordain" (NACM).
The only people who aren't appointed are the monarch, who is hereditary, and Members of Parliament and the Speaker of the House of Commons, who are elected. Everyone else, the Governor General, the Secretary to the Governor General, the Prime Minister, other ministers of the Crown, the Chief of the Defence Staff, deputy ministers, justices, senators, the Speaker of the Senate, the Usher of the Black Rod, etc... are all appointed.
