Good question. Juliet has spent her entire life locked up in her house, being able to go out only to go to church or confession. Her trips to the confessional are the only time she has anything near freedom. For so long as the marriage is kept secret, Juliet is going to be kept under close watch. The only way for her to get out is to tell her parents that she has surrendered herself into the custody of a husband.

I often wondered why, when Juliet goes to confession to get some advice from the Friar, the Friar didn't just disguise her as a nun and ship her off to Mantua. But if he did that, he would have to explain to Capulet where his daughter has gone off to. Of course, she could pretend to be a nun--that's no more dishonest than pretending to be dead.

Anyway, we can assume that the friar and Juliet were reluctant to just up and tell her parents the truth because they would likely think of an easy way to deal with this Tybalt-killer and Juliet-ravisher Romeo in one go. In fact, Lady Capulet talks about poisoning him anyway, without knowing that he has secretly married her daughter. If they knew that, all the more likely that they would do Romeo in.
