"From ancient grudge break to new mutiny". The Prince has passed laws to keep the peace, but because of their feud, the Montagues and Capulets keep on breaking those laws and breaking that peace. Mutiny is disobeying your superiors; the Prince is their superior and has commanded them not to fight, but they disobey him and start fighting again (it is "new mutiny", a new outbreak of violence).

Well the title scene for Prince Caspian occurred during Caspian's escape from the Castle and showed him riding on horse back across some hills, but I'm assuming you mean the beach scene near the beginning where the four children first arrive back into Narnia. That was shot at Cathedral Cove in New Zealand.
The queen was not at all mentioned in the book, but Prince Caspian is still the King of Narnia. At the very end of the book, Prince Caspian marries the daughter of Ramandu, of Ramandu's Island, which is the final island before "the end of the world". Ramandu himself was actually a retired star (i.e. celestial star).
