He had about a dozen nodes removed from his vocal cords in January 1987. He stated this in an interview which you will find out there on the net. To my ears, this has resulted in him losing his "head voice". I don't know about his falsetto; he appears to hoot out a falsetto note still here and there. A lot of his singing of the past, which people are calling "falsetto", is actually head register, which is different. This may have been progressive, as in 1988 and 1989 we was still occasionally ascending up to a high Bb in some songs, but he doesn't appear to do this anymore. It must have gotten harder to do so over time after the surgery. I think he hits one in "Levon" still, which barely comes out. I think his top note is pretty much an Ab these days.
