Up the Mississippi River from New Orleans to some degree. Early jazz already existed in other places, but Chicago became the second major center due to in large part to the Great Migration. New York City has been the Mecca for jazz since the Glory Days of Harlem and bebop.

No. There has, pretty much, always been white musicians in jazz, at least from the early 1900's. Before that, who knows? As there was no recording industry as such, there is no documentary or audi proof of what was going racially in jazz prior to the turn of the 19th century.
