Size is the biggest difference, the Ukulele is smaller and has a much shorter fret board. An ukulele typically only has 4 strings, rather than 6 of a guitar, but you can get them with 8 strings, they double up the notes.

The names of the chords are the same. There are fewer stings, so there are fewer strings to be pressed down. There are some sites that will convert from one to another. If you have a song with one set of chords, a chord chart for the other instrument can make it pretty easy to switch.
