Mike shot himself after a day with an evil game of hide-and-seek with Kyle and Sonya, and a hostage crisis at Kyle's house. Mike believed the time loop was over and he didn't want to face the consequences of his actions.
At the very end, Mike wakes up in Rehab again, "Another day in Paradise." He is still in a loop. I believe he is looping over Thursday (the previous day was Wednesday, over and over). I also believe he will find that Kyle and Sonya are no longer in the loop. They are starting every Thursday fresh, with no memories of the previous one (though they will remember all the Wednesday loops). He is alone, and the only way out is to make his own amends. Or go do more horrible things, but that will get old after a while.

When Jack's using his right hand to choke John, Jack's right hand starts to disappear. While Jack's hand's disappearing, Jack backs away in Horror, but Jack still keeps John's gun pointed at John.
After Jack's hand disappears, Jack sees Frank holding his shotgun. Before Jack shoots Frank, Frank shoots Jack. Jack is finally dead.
John realizes that Frank's alive because Frank stopped smoking 30 years ago. Frank and John have a father-son hug.
Epilogue
John and Frank (in 1999) playing Baseball together as Julia looks on. Since Frank stopped smoking 30 years ago, something prevented John's girlfriend from breaking up with John. John's wife gave birth to a boy.
