Well it was kind of a medley of reasons, one being Lauren Holly wanted to go after different roles, and try more movies as well. Also, Jenny Sheppard had cancer, as is hinted in several episodes, she was not going to last too long anyway, and the producers/directors wanted to end the season with a big bang, which invloved killing someone off. If you think about it, she was the most logical choice, she was, in a sense, the least stable main character. She was a big enough star to stir up emotions in viewers, yet small enough that the show could easily continue.
Jenny, Gibbs and another man were sent to assassinate three members of the mob many years before in France. They all thought it was over until years later when the other man dies and their code word is leaked.
Jenny tried to protect Gibbs; she goes out to kill the one person who was left and gets herself killed. Gibbs figures out that Jenny didn't kill the last member of the mob they were tasked to kill all those years ago in France and now she wants revenge - since Jenny and the other man are dead, the only one left to kill is Gibbs.
Well at the end of season 5 Jenny, Ziva and Tony had to go to LA so Jenny could attend a funeral for an old partner that she was with in the Paris opp. When Jenny figured out that her former partner was murdered she figured that she was next and called in the help of Mike Franks. They followed the dead guys wife's instructions to find his insurance plan out into the desert to an old Diner where some assassins caught Jenny off guard while Mike was out the back getting water. Mike only was able to come in and finish off the assassins after Jenny had been shot multiple times. Tony and Ziva managed to get to the scene after losing the director and after Mike had gone. Sorry if I missed some stuff but that's what I remember.
Jenny Shepard died heroically in a hail of bullets after taking out 4 (count 'em! 4!) baddies in the finale episode of Season 5 but, because she was dying of a disease that would have been very painful and caused her to lose her motor skills, we think it was a form of 'suicide by baddie' (as opposed to suicide by cop). But wait! there's more! She died in order to protect Gibbs.

No, Dr. Donald "Ducky" Mallard did not get killed off in the final episode of NCIS. He retired from his role as the Chief Medical Examiner but remained alive.

It's not specific in the least, which is normal for an NCIS finale. The whole team is in the building when the bomb goes off, and each has a level of exposure to the explosion in some way shape or form. Ducky seemingly has a heart attack on the beach when he heada that the office was bombed. The only way you'll get a clear answer is, of course, when the next season premiers.
Nothing happened to her. She is fine and is very much a part of the NCIS team headed by Jethro Gibbs. In Season 8 she started getting romantically involved with CIA Agent Ray Cruze. But, in season 9, in the last episode she broke it off when Ray killed a Naval officer in place of a terrorist.
