It can be anywhere between 36 and 42 inches.
The most common rail currently used is 141 AREMA with a height of 7-7/16". There are over 20 rails still used in the US (9 still in production) Outside of the Class 1 railroads 115 AREMA is very common with a height of 5-1/2"

39" to 43" is the bar height guideline ,then the height of the top of the rail will be around 6 to 9". It should stick out from the face of the bar to allow room for most of a customer's feet, at least 7 to 8" but should still be close enough to the customer's edge of the bar so that he can reach it. For those customers that like standing , you want the foot rail to be about 6-10" from the outside edge of the bar.
Measure from the nose and the top of the tread of the plumb up the wall 34' to 36'
42 inches from the top of the rail for openings / barricades and 36 inches to the top from the top of a stair tread on stairs.
depends on the size of the rail box
they should average 32" to 36" from the front of a tread or the surface of the porch or deck.
this is simply known as the 'lower dash rail'
