In the early 1960's by the Lockheed Skunk Works. It saw service as a recon aircraft from 1964 to 1998.
I've never come across a name for the skunk, and, strangely, it's never occurred to me to ask. I've had a look and it appears to not have a name, but this is a great question and I'll do my best to find out.
