If the boss was instrumental in getting the raise in salary for you a note (hand written please, e-mail just doesn't cut it here) of appreciation would be in order.
If the increment was a standard thing then the next time you are talking with your boss ( and there is no one else there) a simple thank you will suffice. If you want to let them know that you really want to do more and look forward for more I would suggest you write and email like this one. Dear Sir / Madam (NAME, if you address them by name) Thank you for the generous pay rise that I have received this month. I would like you to know that I view my future at XYZ on a very positive note and having my efforts recognized with a pay rise cements my opinion that our company is a fantastic employer. I will continue doing my best for this Company. On this note, Could I request you to meet for 30 minutes anytime this week or the next to discuss my role and development? If you want to reply to a boss, for them giving you an appreciation mail or letter, you can write them a thank you letter. You can tell them that it's nice for someone to recognize your hard work and dedication.
It is not necessarily customary, but it is polite to thank anyone for anything they do for you if you truly feel that way. However if you just want to appear like you care then I suppose thanking him would be customary. You do not require gifts, just a verbal thank you or maybe a card.
It is usually ok to not send them a thank you card back because they are doing it to show they care, although you can if you really want to.
