Fold a large square of cloth diagonally in half, making a triangle. The elbow goes at the point of the triangle when the two separate tips come together. The two remaining tips -- one goes up the front and the other either goes around the head (if the patient is big or the sling small), or around the back (if there's enough material), and knot they together. Use the knot to adjust the sling so the arm is either perpendicular to the ground, or in whatever comfortable, immobilizing position you want. The remaining point, where the elbow is, can be pinned to the rest of the sling to form a cup to hold the elbow, if you want.
The reason you're using a sling is to immobilize an injury, so that's your design goal when you make one.
Things to avoid or worry about are the victim either being in pain or losing circuloation in the limb. Look for signs of failed circulation (cyanosis, lack of pulse, etc.) and treat that first.
