It's because back then there were no punctuation marks so when people wrote a question they would put the letters "Q" "O" at the end of a question when they wrote letters or stories. So it would look like this: What's for dinnerQO

But when people got the letters, they thought "Q" "O" were apart of the sentence, so then people started writing the "Q" over the "O".

Then after time people became lazy writing "Q" and "O", so then as time went by, the "Q" became eroded and was drawn halfway, and the "O" became a tiny little dot.
