Once you get good enough to do so, you begin speaking a foreign language (in this case including English, if that is not your native tongue) conversationally - and when speaking conversationally, you probably just say the first word you think of, whatever language it's in.
I know French and am learning Chinese, and often, even while speaking in English, the first version of a word I will think of is in one of those languages.
