Anything defined as a CMV will require a CDL, regardless. So if it's pulling a trailer, yes, it will require a CDL.
Technically, yes - you're supposed to be CDL licensed to even be in actual physical control of a CMV. However, you're not going to get a ticket operating a CMV on private property without a CDL. If you get a reckless driving charge in a four wheeler as a CDL holder, it's no different than if you didn't have a CDL.. except you'll probably lose your job and finding work as a CMV operator isn't going to be fruitful for you afterwards. And yes, offences in a four wheeler go on the same MVR as offences in a CMV do, so a ticket you get in your personal vehicle will affect you employment wise, and you're required to report that ticket just the same as you are if you were cited in a CMV. If you got a reckless driving charge while operating a CMV, the only difference between the criteria for that and a reckless driving charge is that the officer has a little more leniency to declare it reckless driving.. and that you'll get hammered on your CSA2010 report, as well.

No... CDL information from all states goes to CDLIS, and you'll be denied in every state. If you did something causing your CDL to be revoked for life, you probably don't need to be out there in a CMV, anyhow...
A CDL is required to operate a CMV in ALL states and territories of the United States. The US recognises equivalent licences from Mexico and Canada, as well.
Are you talking about for a CDL driver, or no? For a CDL driver operating a CMV, you must be at least 18 to drive intrastate, and at least 21 to drive interstate.
