According to the OSHA Occupational Safety and Health Standards (29 CFR 1910) section 141.(c)(l)(i), employers are required to provide their employees with toilet facilities separated for each sex in all places of employment.

The toilet facilities can be public restrooms or portable toilets, as long as they are available within the area of employment and have designations as either a men's or women's restroom. They don't need to be specifically assigned for employee or customer use, and the employer doesn't need to have single person bathrooms.

Most states require an employer to provide a bathroom as well as bathroom breaks. You can file a complaint with your states department of labor if they don't.
