Yes you do. The Sims Medieval: Pirates and Nobles is an add on to the Sims Medieval :)
There is no "royalty" in The Sims 3. There are however nobles/kings/queens and such in the Sims Medieval, which is very much like Sims 3 but during Medieval times.
Well you can't make real pirates on Sims 2, although you can dress them as if they're pirates by making them have pirate style hair and clothes.
This works for basically any sims game: On your home screen, right click the icon of the sims, sims generations, sims pets, ect... Click 'Open file location' This should take you to some files. Scroll down and you should see something called 'TS3' and TS3W'. Click 'TS3' And you're done! :)
