The Dot Physical is one of the requirements that is needed by the commercial bus drivers. They are usually needed after every three month's to check the medical suitability of a given driver.

Check the Yellow Pages (or the Yellowbook website will suffice), find some clinics, call and ask if they do DOT physicals. Keep in mind, also, that if you're running regional/OTR/etc., you can get a DOT physical in any state - they're all federally regulated. My Oklahoma DOT physical card worked with my North Carolina (and, later, Colorado) CDL.
When you go to the DMV to get your CDL, you'll be expected to show either your DOT medical card, or the DOT physical long form. Additionally, any employer hiring you as a CMV operator is going to require you to present your DOT physical card, and possibly the long form physical, as well.

The expiration date should be both on the long form and your DOT medical card. It can be valid for up to 24 months, but can also be for less in cases where age, medical condition, etc. becomes a factor. The expiration date is the day it expires - there is no grace period.
