I wish I knew, it's communism in action. This is strictly meant to make as many people as possible more and more dependent on the Government. For the past few years we have been restricted by Federal Law from writing health insurance for a child. This has always been a normal part of business, to write health insurance for children only. With the current administration, we have been forbidden, so that they could create a crisis and be able to increase the number of children without health insurance.
I just finished a class on the ACA and related laws. There are a ridiculous number of exemptions from being penalized under the law. All American Indians, those receiving coverage from government, household income below filing threshold, incarcerate persons, victims of domestic violence, those who filed bancrutcy, homeless persons, those who received an eviction notice, those who received a shut-off notice from a utility, those with death in their family, and a slew of other exemptions. Another big one is if your State did not expand Medicaid to cover this program you are exempt from the penalties.
One big tell-all is that they are giving subsidies and tax credits to people only who buy or enroll in free coverage from the marketplace. If you buy private health insurance or pay for it through your employer, you are not eligible for these subsidies or tax credits.
The actual name of it is the Affordable Care Act, and some people call it Health Care Reform.

Some doctors will lose money, since the standards will be uniform, care for some will be worse, also has to be funded from somewhere which is bad for the source of said funding.

There are of course a couple of downsides to the health care reform, the main one for most people being the increased involvement of the government in people's everyday lives. Also there is the issue of the mandates that are supposed to be passed along with the rest of the health care bill that would require everyone to have some sort of health care insurance (or get fined) on the basis that it would lower prices for everyone else. Many people dislike this idea. There is of course also the cost of putting the health care plan into action.
The health care reform trying to increase costs
As far as can be determined, the documentary had little impact other than to make people talk about the American Health care system and discuss some of its failings.
Basically, yes. The insurer may not be required to cover "pre-existing conditions" for some period of time, though the health care reform bill is supposed to make some changes to that.
Contrary to some online myths, there is nothing in the health care reform law that gives health care benefits to undocumented immigrants. I enclose a link to the law, so you can see for yourself. That said, congress is in the midst of debating immigration reform, and there may be some opportunities given to children of undocumented immigrants, so-called "Dreamers" who were brought here when they were babies and have lived in the US all their lives. That is not in the current law, however.
It is a plan to make quality healthcare accessable to everyone by making health insurance affordable.
