Marijuana's active cannabinoids do not 'block' the pain. Some research suggests there may be a secondary dopamine and endorphin reaction caused by the 'high'. What many users do find is that the peaceful, euphoric sensation puts one in a position to better endure their illness. Also, many chronic conditions affect the appetite; see cannabis as an appetite stimulant and anti nauseate.

Side note: cannabis increases the effect of narcotic pain killers.

This is very hard to explain, so I will use an example. A muscle spasm is caused by your brain accidentally sending the wrong signal to your muscles. A seizure is also caused by this, but is more widespread to almost all parts of the body. Medical marijuana is used to treat Seizures because it helps extra oxygen travel to just about all of the brain. This helps to prevent brain signals from going to places they aren't supposed to.(the most simple way I can explain this)
