I've used fentanyl patches in the past for chronic pain, but was not opioid-tolerant enough for them to be appropriate until now, several years later. Regarding the question, based both on experience and info I've found on the web, it takes fentanyl patches about 12 hours to begin working, and about 24-28 hours for peak pain relief to start. And, as I'm sure you know, they should last for (and one should only use one every) 2 - 3 days.
I had a broken pelvis and when the I.V. pain killers stopped after my hospital discharge the Doc put me on Fentayl patches. They are very subtle, I know that they are way strong and way addictive. But, the vicodin seem to work better. Then again I was on both at the same time so I'm not sure. But, please be careful with thoes Patches. And remember to cover it if your getting in the shower. Or it will fall rite off.

When you first start using the Fentynal It takes about 24 hours to feel the full effect but can take up to a week. I have been on Fentynal for 2 years now and find it very effective. I started at 25mcg/h patch and just recently started the 100mcg/h patch ( Rheumatoid arthritis and Fibromyalgia) They are better then taking a pill because they don't destroy the liver because they pass right into the blood stream/
It depends on the user, but typically between 3-6 hours after you put it on you'll really start to feel the effects, and 48-72 hours for full dosage delivery. Keep in mind that also depends on patch placement, body temperature, and how well the skin is cleaned/able to transfer the dosage transdermally. Make sure you're not using it on a part of your body that's hairy, or you'll have problems (not to mention it won't stick for very long).
It's important to note for first time users that initial patch dosages are based on morphine/oral opiate conversion tables (to Fentanyl), and those tables are intentionally skewed toward the low side for safety reasons. It's not uncommon for initial users to start with a patch that's too low a dose.
It depend on the patient, patch dose, patient's opiate tolerance, and activity level, but in general with 2-4 hours. Of course it's typical for first time users to get a patch dose that's lower than the equivalent dosage of Oxycodone or other opiates previously taken - this is a standard safety practice when beginning patches. If you're not feeling any effect after 6 hours, the dose is probably too low and needs to be adjusted.
