The first things affected by alcohol are judgment and the ability to make good decisions -- what psychologists call executive functions. This occurs before you begin to "feel the drinks," and is the reason why people continually make bad decisions about driving after drinking. Alcohol also affects behavior as a driver, which also requires judgment and good decision-making.
Most people think it has to do with reaction times and things like that. If only it were true. It is possible to drive safely with poor reaction times and physical abilities, if your brain is working well. Turn off the brain, and everything goes south.
It's not a choice generally made under the same rationality your mind normally follows. Alcohol numbs the mind down to it's more basic functions, while also boosting self confidence, and removing the normal apprehension for activities you might not otherwise engage in.
That is to say, people are still making the choice to get in the car and drive. But their mental faculties are often not functioning as they should be. And thus what seems like an alright idea is actually a terrible one.
There is also "buzzed" driving ,where one isn't drunk but still shouldn't be driving. As the law doesn't see there being a difference. In this case the mind is still functioning fairly well. Thus it becomes a case of overconfidence in ones own sobriety. Often in these cases there may be other additional motivational factors involved. Such as the girl you just met that looks amazing and wants to go home with you.
In the end it all just boils down to bad decision making, regardless of your mental state. Personally I've always lived by the motto of "No matter what drug or drink I may be on, I am at all times responsible for my own actions".
