For Type 1 Diabetics, extended sugars of over 250 require testing for ketones. In Type 1 Diabetics (and some Type 2) a lack of insulin can cause the cells to go crazy producing ketones. High ketones are deadly and can cause Diabetic Ketoacidosis (DKA) which is fatal if left untreated. That being said, the body doesn’t necessarily produce ketones every time it’s high and you can actually go into DKA with normal blood sugars, it’s jut that one usually comes with the other.

Extended high blood sugars, though, will cause long term damage. The most recent studies that I know of say that the goal should be a a1c of 7.0 or below (average blood sugar of about 154). Time in range should also be taken into account, though. It’s not healthy to healthy to have your blood sugars rollercoastering between 54 and 254 either, even though it would give you an a1c of 7. As always, though, a doctor should always be consulted because there could be other health factors that could play a role as well.
