The affected lung would collapse or not be able to expand fully, so lung ventilation would decrease.
It's not unheard of for Lung Cancer to spread locally to structures nearby such as the heart. It would make it's way in from the outside so getting to the Heart (or Pericardial) Sack first

Emphysema does not "spread"-- it is NOT contagious.However, the incidence of emphysema can be reduced if all countries and persons reduce lung irritants and air pollution. ANY strong chemical is a lung irritant. Smoking is just one of a long list of lung irritants.
The two pleural membranes should be touching. If air creates a space between them the air will push the lung down and prevent it from filling with air. This is known as a collapsed lung, or pneumothorax. The air between the pleural membranes must be removed to reinflate the lung.
Diaphragm cancer, or any cancer that is classified as stage 3 means it has spread to the lymph nodes. The lymph nodes accelerate the spread of the disease.
The mediastinum
The left lung has 2 lobes due to the space occupied by the heart, which pushes against the left lung. The right lung has 3 lobes because it has more space in the chest cavity to accommodate the extra lobe.
Your left lung is smaller. The right lung has three lobes: superior, middle and inferior. The left lung has only two lobes: the superior and inferior. The heart takes up the space that would have otherwise been used for a middle lobe on the left lung.
