No sure what's more common but I had ovarian torsion and I had relatively mild symptoms on and off for about 4-5 weeks. Then I woke up one morning in terrible pain and knew I had to go to the ER right away. My ovary had twisted around itself completely a few times and had to be removed. I don't think the blood supply had been completely cut off until the day when I woke up with extreme pain but it had been twisted and the blood supply had been decreased for quite a while before that.
