A doctor can order any test he or she thinks appropriate. But you cannot be forced to take the test unless you are a child and your guardian or parent agrees, or you are an adult declared to be incompetent on medical matters and have a guardian assigned who agrees to the test.
You should always be aware of what tests are ordered and how they are likely to benefit you. Then you can decide whether or not to have them done.

If the doctor has a reason to check for drugs he or she can order such a test. However, it is not a routine part of a physical and generally is not done without the knowledge and consent of the patient.

NO! The doctor would get in VERY BIG trouble if he/she did. However, if the doctor sends you to have bloodwork or urinalysis done for some other condition, they may well have you tested for pregnancy. It would effect how you are treated for other conditions. And it's not that they're not telling you--you need to ask what the various test codes mean.
no, its actually a lottery process
yes they can and they can do it without you knowing

Ask your doctor to interpret the test results for you. Your doctor may also wish to order additional tests.
