I believe you will find a caveat in the fine print of many coupons that says "Redeemable at participating dealers" or something similar. This would indicate that not all dealers are necessarily participating in the promotion. The reason for this is that, in some cases at least, the dealer is required to buy a certain quantity of stock to qualify, so smaller dealers may opt out.

Any store that accepts manufacturers coupons will allow you to "stack" a manufacturers coupon and a in-store coupon on the same item but in some stores you can even stack an ad-perk coupon on top of a manufacturers coupon and in-store coupon.

Stores definitely reserve the right to refuse a coupon. Most of them will only refuse if they think they are fraudulent though. But they don't have to take any coupon if they don't want to.
