Most probably nothing.

The same enzym in the liver is used to metabolize the grapefruit as well as the citalopram, so a slight increase of the citalopram-level could occur.

But one grapefruit every now and then shouldn't be a problem I guess.

Eating grapefruit while on citalopram can potentially increase blood levels of the medication, leading to a higher risk of side effects including irregular heartbeats or serotonin syndrome. It is best to avoid consuming grapefruit or grapefruit juice while taking citalopram.
