It would make more sense to see the movie with the origin story for Batman first. The Scarecrow has a cameo at the beginning of the second film, so the viewer would be more familiar with what is going on if they had already seen him as an important character in the first film. The film is probably able to be followed okay by itself, but it is a sequel expecting the viewer to already have seen the original.

Batman is Bruce Wayne, who traveled the world and developed his body and mind through sheer willpower, focus and knowledge, whereas The Joker is an unnamed individual with no genuine origin who suddenly appears and starts killing people for reasons that only make sense to him. He also represents escalation. More specifically, the escalation of Batman. Whether or not the next step in putting on a costume and beating up the bad guys is Batman himself becoming an outlaw or a even a psychotic. He's the fear of chaos to Batman's organized crusade. These are all classic themes presented throughout Batman lore, as well as in the recent picture The Dark Knight.
