In the series, they never reveal his actual name. At the end, Olivier asks him his real name but Scar wants to give up whatever name he secretly had and the nickname "Scar" because he says he's giving up his past lives or whatever, and Olivier just ends up calling him Ishvalan.

They never mention how Scar got the name "Scar" in the film series other than possibly his parents named him that. However, in The Lion King: Six New Adventures book approved by Disney, his birth name was Taka, but that is not the movie Scar's real name because it was denied by the filmmakers on the audio commentary found on the DVD releases of The Lion King, when they denied the cub at the end of the film was Kopa, the main character of the book.
They never mention what Scar's real name was in the film universe. In a series of books approved by the general Disney, his birth name was Taka but that is not the movie Scar's real name because it was not approved by Scar's creators as in the first film's makers - as is evident from the commentary track of the film. That name is barely semi-official, because even though Disney gave its stamp on the books, that name was made up by a man who wasn't part of Disney. As in that Disney only holds copyrights of the name, it didn't come up with it. And as said, it isn't the movie Scar's real name. His real name remains unknown.
