Equip cards can never remain face-up on the field without their target monster. If the monster leaves the field or is turned face down, then the Equip card will be destroyed. That goes for regular Equip Spell cards, but also monsters stolen and turned into Equips by Relinquished, etc.
However keep in mind that cards like Call of the Haunted do not actually 'equip', so can remain on the field under certain circumstances, like if the summoned monster was tributed instead of destroyed.
In general, a monster's effect can only be activated while it is face-up on the field. The only time they can do otherwise, is if they specifically say so. So Necro Gardna has an effect that activates in the graveyard, and DD Survivor has an effect that activates in the removed from play zone, for example. But you could not activate the regular Ignition Effect of, say, a Judgment Dragon in the removed from play zone.

No, Equip Cards simply 'fall off' when the monster they are equipped to is destroyed. They do not go to the graveyard, they are now just unattached.

As long as there is a valid target, Flint will trigger and re-equip to a monster after the previous equip target is destroyed. That may mean it has to equip one of your own monsters, you can't just decide not to allow it to come back, it does so as a mandatory effect as long as there is indeed something it can equip to.
